Chronic Care Guidelines
Licensing the evidence-based Chronic Care Guidelines provides a robust framework for disease or care management of chronic, complex, or multi-diagnosis members. In addition, each of the 24 chronic conditions covered includes a set of patient education materials that are written at a fourth-grade level, designed to be sent to a member as part of an intervention. This valuable material is available in both English and Spanish, to serve more members.
